You can request an appointment with a doctor, nurse, healthcare assistant, clinical pharmacist or physician associate by coming into Claremont Clinic in person and speaking with reception, calling reception on 020 8522 0222 during working hours or by using the online consult form below.

• Please note that the online consultation forms are available Monday to Friday between 8am and 6.30pm excluding bank holidays.
• Claremont Clinic will aim to action your completed form within 2 working days.
• All online consultation forms submitted are securely linked into our clinical systems.
• Please ensure you are available on the phone number you have provided as we will use tis to contact you by call or by text message (SMS)
• Please let us know if you require a translator for an appointment as this can be organised.
